Collect all of the pages of your form as a single PDF. You can manually fill out the form prior to upload, or leave it blank and complete it on the Proof platform. However, do not sign the form before uploading. You must sign the document during the notary session.
If your form requires more than one signer, you will both need to be present in the same room to meet with the online notary. Click here for directions for multiple signers who cannot meet the notary together. Please note that this method has the highest total fee due to the separate notary sessions.
In seconds, Proof will connect you with a commissioned notary over a live video feed. The notary will confirm your identity, witness your signature, and assist you throughout the process.
After the notary meeting, Proof will prompt for credit card payment before you can access your completed forms.
