Transamerica has partnered with Proof to offer a secure way to notarize documents online. Proof offers 24/7 access to commissioned online notaries, allowing you to complete your forms at your convenience – anywhere, anytime.

‍If your form requires a medallion signature guarantee: Transamerica will accept a notarization through Proof as an alternative to this requirement.

If your form requires a signature from your employer or plan administrator: Your form cannot be submitted through Proof. You will be required to obtain the additional signatures and submit the form to Transamerica.

How it works

1. Upload your form

Collect all of the pages of your form as a single PDF. You can manually fill out the form prior to upload, or leave it blank and complete it on the Proof platform. However, do not sign the form before uploading. You must sign the document during the notary session.

2. Verify your identity

Proof will ask you a series of questions and prompt photos of a valid, government-issued ID to quickly and securely verify your identity.

3. Connect with a live notary agent

In seconds, Proof will connect you with a commissioned notary over a live video feed. The notary will confirm your identity, witness your signature, and assist you throughout the process.

4. You're finished!

After the notary meeting, Proof will prompt for credit card payment before submitting your completed form to Transamerica for review. Transamerica will typically process your request within three business days. Please do not mail or fax a copy of this form to Transamerica.

Here's what you need

ID #1: The Photo ID

The Photo ID must be unexpired with a clear photograph. Acceptable Photo IDs include:

  • State-issued Driver’s or Nondriver’s Identification Card
  • Passport
  • U.S. Permanent Resident Card issued by U.S. Citizenship and Immigration Services
  • U.S. Certificate of Citizenship or Naturalization
  • Matricula Consular (Mexico)
  • NEXUS Card (Canada)
  • U.S. University Identification Card

Please note: Customers without a Social Security Number are required to notarize through the online notary and must use a Passport as their Photo ID.

ID #2: The Address ID

The Address ID must match the address listed on Form 1583 (4f-j and 9b-f). Acceptable Address IDs include:

  • State-issued Driver’s or Nondriver’s identification Card (Unless used for Photo ID)
  • Voter or Vehicle Registration Card
  • Home or Vehicle Insurance Policy (Declarations page is suggested)
  • Lease, Mortgage, or Deed of Trust (*See note below)
  • U.S. State/Territory/Tribal Card (Federally or State recognized tribal nation)

* Leases: include name, address, start, and ending date, and signature pages.
* Deeds: include name, address, and signature pages.

Here's what you need

  • A single, unsigned PDF that includes all form pages
  • A valid form of ID
  • The last four digits of your U.S. Social Security number
  • A credit card to pay the $25 notarization fee

Single Participants

Married Participants
Plan participants should download the spousal waiver form and have their spouse complete this process.
